OPERATORS <br /> OF SOUTHERN CALIFORNIA <br /> Designated UST Operator Services for(Gasoline RetailecuEiVrD <br /> JUN 16 2014 <br /> ENVIRONMENTAL HATH <br /> May 28,2014 PERMITISERVICES <br /> RE:Pilot/Flying 1 #618 Ripon <br /> To Whom It May Concern, <br /> Per CUPA Inspection Report dated May 14,2014,Violation 204.The L-6(Diesel 4 STP Sump)alarm at <br /> 17:35pm on Feb 27,2014 was caused by water intrusion during heavy rains passing through the state. <br /> Other sumps that alarmed during the storm were: <br /> L-22(Disp 29 Sat) Feb 28 at 5:45am <br /> L-24(Disp 30 Sat) Feb 28 at 4:20am <br /> L-29(Disp 19 Sat) Feb 28 at 11:59am <br /> Site management failed to specifically list the L-6 alarm on the Alarm/Response Log sheet and the <br /> Designated Operator missed investigating the cause of this alarm on the DO Report for March 2014.The <br /> L-6 alarm was'checked off'on the original DO Report-Alarm History section.After further investigation <br /> and questioning of site management it was determined to have been caused by the heavy downpours <br /> passing through the area on the 27th and 28th of February. <br /> Site management was reminded of the importance of recording every alarm with its appropriate <br /> response on the Alarm/Response Log sheet.The Designated Operator is also reminded of the <br /> importance of investigating and recording each and every alarm. <br /> Best regards, <br /> Lance York <br /> Phone: 949.458-USTO (8786) Fax: 949.949.589.2546 Ce11:949.230.8931 www.ustoperators.com <br /> 21352 Silvertree Lane, Trabuco Canyon,CA 92679 <br />
