Site is equipped with Gas, Auto Diesel, Truck Diesel, Bio Diesel and Oil
Water separator UST's. Tested Gas, Auto diesel, bio diesel trans sumps and truck diesel Liquid sensors to include STP,
Annular and UDC's. UDC 16 sensor was frayed and replaced per inspector. Like for Like 794380-208 and witnessed by
inspector no permit required. T3 Diesel annular failed to alarm replaced with a new like for like 794380-303, retested good
inspector witnessed no permit required. Also tested fail safe, Sensor out and spill buckets. T1 diesel, T2 diesel, T3 diesel and
T4 87 spill buckets failed initial tests. T1 and T2 4 due to drain valves leaking. Replaced both with new Phil -rite 85400 drain
valves, retested good. T1 also required a new swivel adapter, replaced with a new OPW 61SALP. T2 and T3 spill buckets were
found to be leaking into spill bucket secondary space. Removed all drains and harware, replaced o -rings and resealed all
components. Retested good, but inspector was only on site to witness T3 pass, as a result site will not be allowed to receive
fuel drops in tanks 1 or 2 until a witnessed inspection takes place. Will return on 5/6/16 to complete Bio and water oil separator
certification and to complete witnessed test on tanks 1 and 2 Spill buckets. Placed Pad locks on the fill caps of the tanks that
are not to receive fuel drop as an added precaution. Pad lock type are the same locks Jones Covey uses to lock Pilot sensors
into place.
5/5/2016 - On site for witnessed spill bucket test on 1 and 2. With use of INCON performed hydrostatic test on 1 and 2.
Inspector Aris only required 1 15 minute test. Both buckets passed without issue. After testing the integrity we verified that both
buckets hold a minimum of 5 gallons. Per Inspector the site may now receive fuel drops.
5/6/2016 on site to complete Annual Monitor Certification. Tested Bio Diesel components to include liquid sensors and vacuum
sensors. Tested communication on all vac sensors as well as brine monitored sumps. Tested all leak detectors to include Gas,
Auto diesel and Diesel. Also tested oil water seperator sensors and spill bucket. Diesel dispensers 19 and 24 had leaks on
hanging hardware, both repaired by Jose Vega site RMT, he also cleaned out all Dispenser UDC's. Liquid/water was found in
STP sumps and transition sumpswas cleaned out and placed in a hazardous waste drum with proper labeling. it was noticed
that there are some leaks in the Bio shed appears to ge Flanges and unions, leaks are before bypass valves_ Site may also
have issue with their monitoring plan, currently they Drop Tube flappers as their overfill prevention but the current monitoring
plan also shows the use of a audio/visual alarm. Also Inspector Elena noticed the Gas dispensers fuel filters are mounted in a
position of which they were to leak the dripping product may not leak into the UDC as required. Site may need to make use of
a slopped drip pan.
794380-208 Veeder Root Sump Sensor - 12 ft cable
1.00
794380-303 Veeder Root dual point hydrostatic sensor 1.00
Page 1 of 2
