There are three items missing that she would like to be filled out and returned to her. <br /> Elena is requesting the following. <br /> 1. Service of request for billing information, one of the two boxes needs to be checked off. <br /> 2. Page two,the retrofit billing information at the bottom of the page is empty and needs to be filled out. <br /> 3. The sump repair, she needs to know the make and model of the current sump. <br /> Would you please take care of these three points and send the information to her and make sure to CC me in <br /> that email.
