Banner,Lodi CA 95205 (SR0056210) <br /> The San Joaquin County Environmental Health Department(EHD)has completed the <br /> review of the application to addendum the permit to include the replaced VR-208 diesel <br /> STP sump sensor in addition to the VR-208 premium STP sump sensor at the above <br /> noted address. Both sensors have already been tested to pass in the presence of the <br /> inspector on 3-4-09. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested, repaired, or serviced, and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 3. Permit expires 180 days from date of approval. Note: If an extension to the <br /> expiration date is required, a written request must be submitted to this office prior <br /> to the expiration date. <br /> 4. All components must be certified by an independent testing organization and <br /> labeled as required by the organization. (UL, ULC, ANSI,API,ASTM, etc. See <br /> California Code of Regulations(CCR), Title 23 Chapter 16 Table B for complete <br /> Page 1 of 2 <br />
