Grantline Road,Tracy, 7-Eleven 2237-32262 Your application for a permit to install a new underground storage tank system at the above location has been approved with the following conditions: 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for applicable permits. 2. Any changes shall be submitted to the San Joaquin County Environmental Health Department (EHD) for approval prior to commencing work. All new submittals supercede the previous ones. 3. If the underground storage tank and associated piping are not installed by July 1, 2003,then the new monitoring system requirements and construction requirements under Assembly Bill 2481 must be satisfied. An addendum to this permit must be submitted to EHD and approval must be received by EHD prior to commencing work. 4. Both primary and secondary pipes shall be UL listed and bear a UL label. 5. All pipe fittings and connections shall be UL listed and bear a UL label. 6. All underground storage tanks shall be UL listed and bear a UL label. 7. All monitoring systems shall be listed on the LG113-16. 8. All testing shall be in compliance with manufacturer's instruction, section 2635 and section 2636 of California Code of Regulations (CCR). Automatic pump shutdown must satisfy CCR 2636(g). 9. All piping must slope to a containment sump with an audible/visual alarm. 10. All equipment in contact with backfill must be corrosion protected. 11. Overfill prevention must satisfy CCR 2635(b)(2). 12. An accurate and drawn-to scale-as-built plan must be submitted to EHD prior to the final inspection. 13. A complete monitoring and response plan must be submitted prior to the final inspection. 14. All lines and tanks shall be tested for tightness, in accordance to manufacturer's guidelines by a licensed tank tester, after the tanks are in operating condition and results received prior to the final inspection. 15. A"Certificate of Compliance for Underground Storage Tank Installation Form C"pursuant to California Code of Regulations, Title 23,Underground Storage Tank Regulations, section 2635 (d) (copy of section attached) shall be submitted to EHD prior to facility being issued an operating permit.
