6. Since concrete will be broken near the 91 (premium) UST, a tank integrity test and line <br />integrity test (or equivalent) must be performed, once concrete is poured in accordance <br />with the manufacturer's guidelines. Results must be received by EHD within seven days <br />of returning the facility to operating condition. <br />7. Contact Michelle Henry (953-7699) to arrange the following inspections (a 48-hour <br />notice is required): <br />a. EHD must witness the functionality of the monitoring system to ensure that <br />monitoring has been restored to meet the requirements of California Code of <br />Regulations Title 23 and the Health & Safety Code. <br />8. The monitoring must satisfy this facility's approved monitoring program. <br />9. An "Underground Storage Tank Certification of Installation/Modification" form shall be <br />submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before this <br />upgrade will be considered complete by EHD. <br />10.
