The following is a <br />Class II Repeat Violations: 311 <br />311. (Repeat) The 87 -octane line leak detector failed to detect a leak when tested. All line leak <br />detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician <br />adjusted the leak detector, retested it, and it passed. <br />NOTES (following are not violations) <br />104. Current financial responsibility documents is due on May 23, 2014. Current financial responsibility <br />documents are required to be submitted annually. Immediately submit a copy of the financial <br />responsibility documents to the EHD before the deadline. Alternatively, log into CERS, upload the <br />required information, and submit for review by the EHD. Please be aware uploading to CERS will be the <br />only alternative in the future (see Below) <br />Facility operators are required to upload the following UST program documents into the California <br />Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial <br />Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST <br />Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance. <br />After January 1, 2015, noncompliance with this requirement will be a violation. <br />Submit the following to EHD by: 06/19/2014 <br />1) A completed copy of the (provided) Return to Compliance Certification (please keep original for <br />your records). 2) A written statement how each violation was corrected. 3) Evidence (i.e. photos, <br />disposal receipts, documents) in support of the corrections made. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
