The July 2011 designated operator monthly inspection report was not found on site. Designated <br />operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br />ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit a copy of the 7/2011 report to the Environmental Health Department (EHD) <br />by June 1, 2012. <br />311. The 91 octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br />obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br />immediately, there is a possibility that the 91 octane UST system may be red tagged to prevent fuel <br />inputs. <br />Note <br />1. Complete the following by June 1, 2012 and send to the San Joaquin County Environmental Health <br />Department: <br />a. The Return to Compliance Certification form. <br />b. A statement explaining how each violation was corrected. <br />c. Supporting documentation. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REI TION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
