Please find below a list of the violations noted at the <br /> referenced facility and Pilot Flying J's (PFJ) responses. <br /> 1. Plot plan/map not submitted or failed to completely show where monitoring is <br /> performed. An accurate UST Monitoring Site Plan was not submitted. The UST <br /> Monitoring Site Plan map key does not identify what L1 through L25 are for. A site <br /> plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System (CERS) and <br /> upload a copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> The requested updates have been made to the Monitoring Site Plan in CERS. <br /> 2. Designated operator did not inspect all required items and/or the inspection reports <br /> were not completed. The designated operator failed to check and note the correct <br /> test dates for the monitoring system certification, spill container, and mechanical line <br /> leak detector tests on the 10-22-15 designated operator monthly inspection report. <br /> The date for these tests were listed as 5-3-15. These test dates should have been <br /> documented as 5-13-15. During the monthly inspection, the designated operator <br /> shall check that all required testing and maintenance for the UST system have been <br /> completed and document the dates they were done.
