Lodi CA 95242 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br /> 2. Secondary containment testing was performed on 3-3-11 and a test report has not been submitted to <br /> the EHD. A copy of the test report must be submitted within 30 days of the test. Immediately submit a <br /> copy of the test report to the EHD. <br /> 7. Maintenance records for the L45 transition sump 1 high liquid alarm on 8-2-10 were not found on site. <br /> These records shall be kept on site for at least 3 years . Immediately locate and maintain the <br /> maintenance record for the L45 transition sump 1 high liquid alarm on 8-2-10 on site and submit copies <br /> to the EHD by 6-5-11. <br /> 13. The UDC 9/10 and Main 30 UDC sensor failed to activate an audible and visual alarm when tested <br /> to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br /> activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br /> The Veeder Root 794380-322 sensor was replaced and retested during the inspection(UDC 9/10 <br /> SN 120103 and Main 30 UDC SN 120050). Obtain a permit from the EHD for this work within one <br /> business day. If a permit is not obtained within one business day, a "Failure to Obtain a Permit" penalty <br /> will apply. <br /> Note: When Veeder Root 322 sensors fail, we discourage use of the polymer strip sensor discriminating <br /> option due to concern about the impact that repeated prolonged exposure to hydrocarbons to the sensor <br /> will affect its ability to detect, alarm, and recover from an alarm. If installed, ensure that it is programmed <br /> not to discriminate between fuel and liquid. <br /> 17. The tank#2 diesel line leak detector failed to detect a leak when tested. All line leak detectors shall <br /> be capable of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, <br /> trained, and certified contractor repair or replace the failed leak detector (LIS 113-x listed, if applicable) <br /> and obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br /> immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br /> 23c. The designated operator failed to document and failed to document that an alarm was responded to <br /> appropriately. The missing alarms include: L45 transition sump 1 high liquid alarm on 8-2-10. During <br /> the monthly inspection, the designated operator shall review the alarm history for the previous month, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report.
