• • <br /> (ii) A completed certification of financial responsibility must be submitted prior to the <br /> final inspection. A copy is enclosed. <br /> (iii) A completed form C must be submitted prior to the final inspection. A copy is <br /> enclosed. <br /> (iv) Submit an As-Built plan to EHD at the time of the final inspection. Note: A final <br /> inspection will not be performed unless the as-built is submitted. <br /> (v) Monitoring Certification. <br /> (g) Enhanced Leak Detection(SR0038351): notify this office 48 hours prior to install and to <br /> perform the tests which include,but are not limited to,pre test,re-test, and actual test. <br /> (i) Before the underground storage tank is placed in use, the underground storage tank <br /> shall be tested after installation to demonstrate that the tank is product tight. <br /> (ii) A copy of the enhanced leak detection test report shall be submitted a to this office <br /> within 60 days of completion of the test. <br /> 9. Secondary containment systems installed on or after January 1, 2001 shall be tested upon <br /> installation, 6 months after installation, and every 36 months thereafter; result shall be <br /> received by this office within 30 days of the completion of the test. <br /> 10. This permit expires on the June 30,2004 if the tanks and associated piping are not <br /> placed in the ground. Please note that this review is based on HSC 25290.2. for <br /> underground storage tanks installed on or after July 1, 2003 and before July 1, 2004. If <br /> underground storage tanks are installed on or after July 1, 2004, a new design for new <br /> requirements in accordance with HSC 25290.1. shall be submitted for approval prior to <br /> placement of the tank and associated piping in the ground. <br /> If you have any further questions, please contact me at (209) 468-9850. <br /> Thanks, <br /> A& '2 C <br /> Steven C.
