An EPA generator number must be submitted for this tank site. If San Joaquin <br />County has a generator Identification Number issued by Cal -EPA Department of <br />Toxic Substances Control for this site, please provide this information. If a <br />one-time identification number is issued please submit this number to <br />Environmental Health. <br />2. The TANK TRACKING DISPOSITION LOG must be completed within 30 days of <br />acceptance of the tank by the disposal or recycling company. <br />3. A permit must be obtained from the local or County Fire Jurisdiction. Please <br />check with both the local and County Fire Prevention Offices. <br />4. All subcontractors shall have completed current training in order to fulfill <br />the worker protection standards in Title 8 California Code of Regulations. <br />5. Product line tightness test for April 1991, indicate that the product line <br />was not product tight. As discussed on June 3, 1996, a determination of the <br />final disposition of the stockpile will be made after laboratory analysis of the <br />soil samples. <br />6. Laboratory analysis of soil samples shall include the compound Methyl - <br />Tertiary -Butyl -Ether (MTBE). <br />7. The excavated tank shall be transported under manifest as a hazardous waste. <br />A completed copy of the manifest shall be submitted to this office within 30 days <br />of the removal. <br />8.
