SAN" J D�l+ �� I 11411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Super Store Industries-Grocery Division 1 16888 McKinley Ave Lathrop Aril 29 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 714 CCR 66273.36 UWH failed to train employees and maintain records on UW handling and emergency procedures. <br /> Based on the following types of violations observed, the facility employees are not trained on proper universal waste <br /> (UW)handling or the training provided has been inadequate: <br /> -An open cardboard box containing five fluorescent lamps was found not properly labeled in the Universal Waste <br /> Accumulation Area(IMG 5714-5). <br /> -Aerosol cans are punctured and drained and no operating procedures have been developed and implemented. <br /> -Employee training records on UW handling and emergency procedures for the last three years were not available at <br /> the time of inspection. <br /> Employees who handle universal wastes(UW)must be trained initially and annually thereafter. At a minimum, <br /> personnel who manage UW shall trained and thoroughly familiar with proper universal waste management and <br /> emergency response procedures relative to those persons' responsibilities, including where applicable, <br /> 1.types and hazards associated with the UW managed at the facility <br /> 2. proper disposition of UW managed at the facility(e.g. locations of UW containers, or location of centralized UW <br /> area) <br /> 3. proper procedures for responding to releases of UW including the position titles and the means of contacting <br /> those personnel at the facility who are designated to respond to releases <br /> 4. labeling, collecting, handling, consolidating, and shipping UW at the facility, including, but not limited to the <br /> prohibition on the disposal of UW <br /> The UW handler shall maintain a written record by date indication the names of personnel who received the training <br /> and maintain those records for at least three years. <br /> Immediately provide employees with initial or refresher UW training. Submit a copy of the roster and the syllabus to <br /> the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> - In 2018, the facility generated 4.05 tons of hazardous waste. Permit fees will decrease accordingly. <br /> - In the outdoor chemical storage area, an unlabeled container of unknown liquid was found (IMG 5679-81). <br /> According to Scott, the container contained new oil, but directed his employee Regi to dispose of the oil. Regi <br /> poured the oil from the unlabeled container into the hazardous waste used oil poly drum at the time of inspection. <br /> - Manifest 009901145FLE (8/10/2016)was found on site at the time of inspection, but is not on DTSC's <br /> Hazardous Waste Tracking System.
