DATE: August 20, 1992 <br /> SUPERSEDES: N A <br /> APPROVED BY: / DATE: August 20, 1992 <br /> The current SITE MITIGATION PROGRAM Guidelines for Work Plan and Renort Submittal <br /> requires that the "Site Mitigation Acknowledgement/Request for Services Form" (EH <br /> 29 01) must clearly identify the names and addresses of the responsible party to <br /> whom billing information is to be sent. Problems have arisen with collection of <br /> charges billed by PHS/EHD due to changes in the billing party after the <br /> assessment work has been completed but PHS/EHD oversight has not been completed. <br /> This policy stipulates that only the following may be identified as a responsible <br /> party for billing: <br /> a. The owner(s) of the property on which environmental assessment or <br /> investigation activities are being conducted. <br /> b. The operator(s) of a facility on which environmental assessment or <br /> investigation activities are being conducted. <br /> C. A client who has hired an environmental assessment firm to conduct <br /> assessment or investigation activities, but has no ownership of or <br /> responsibility for the site. <br /> If a client or operator who is not a responsible party or <br /> owner is identified as the party responsible for payment of <br /> charges associated with PHS/EHD activities, an authorization <br /> from the property owner must accompany the Site Mitigation <br /> Acknowledgement/Request for Services Form. This authorization <br /> shall state that the property owner is aware of the assessment <br /> activities being performed and may be responsible for payment <br /> of PHS/EHD charges if not paid by the client or operator. <br /> d.
