For <br /> sites with other government agency lead authority, a three (3) hour prepayment of 5159 is required along <br /> with a completed "Request for Services-Site Mitigation Acknowledgment" form. The 'Billing/Responsible <br /> Party" identified on this form will receive billing on a quarterly basis for all services and fees charged to <br /> the site. <br /> Recent correspondence with Mr. Rich Henderson of Golden West Environmental, indicates that this <br /> company will accept billing responsibility and pay the required prepayment in order to expedite <br /> procedures.
