CA ARNG Field Quality Assurance Project Plan <br />9.1.9.17 The COC record (Figure 9-9 at the end of the section) will be used to document the samples <br />taken and the analyses requested. Information that field personnel will record on the COC record (in <br />indelible ink) includes the following: <br />• Installation name; <br />• Project name, event, and charge number; <br />• Sampling location or well designation; <br />• Name and signature of sampler(s); <br />• Dates and times of collection; <br />• Sample designation (grab or composite); <br />• Signature of individual's involved in custody (including dates and times of transfer); and <br />• Airbill number, if appropriate. <br />9.1.9.18 Corrections may be made by drawing one line through the incorrect entry, entering the <br />correction information, and initialing and dating the change. Unused lines on the COC record will be <br />crossed out ("Z"). COC records initiated in the field will be signed, placed in a resealable plastic bag and <br />taped to the inside of the shipping container used for sample transport. Signed airbills will serve as <br />evidence of custody transfer between the field sampler and courier as well as the courier and laboratory. <br />Copies of the COC record and the airbill will be retained and filed by the sampler prior to shipment. <br />9.1.9.19 Upon receipt of an ice chest, laboratory personnel will review the contents and will sign and <br />retain the COC record and the airbill. Information that will be recorded on the COC record, or another <br />appropriate document (such as a laboratory logbook), at the time of sample receipt will include the <br />following: <br />• Status of custody seals; <br />• Temperature of interior of ice chest upon receipt; <br />• Identification number of broken sample containers, if any; <br />• Description of discrepancies between the COC record, sample labels, and requested analyses; and <br />• pH of aqueous samples (not including those containers for VOC analysis) to determine if samples are <br />preserved properly. <br />9.1.9.20 Laboratory personnel will contact the analytical coordinator regarding discrepancies in <br />paperwork and sample preservation and will document nonconformance and corrective actions according <br />to laboratory standard operating orocedures (SOPS). <br />field QAPP.DOC 9-18 May 2000 <br />
