I've reviewed the work plan and application, but I have multiple questions which need addressing before I can issue the <br />permit (or permits): <br />1) Please provide the title / identifier where the document "Stockton QAPP WS #17" (QAPP) came from. There is <br />no reference of the author or the date of the work plan. <br />2) QAPP Section #17c line 927 is not acceptable. Sealant material must be neat cement in area of known or <br />suspected contamination per SJC Well Standards 13.17.6. Also, manufacturer of bentonite does not guarantee <br />bentonite integrity in the vadose zone above the water table <br />3) All materials introduced from the temporary well must be removed before grouting <br />4) Two APNs are listed on the well permit application, while the map of the sampling location only displayed one <br />parcel. Are there more borings than what is listed on the QAPP and map than on the one parcel on Stimson? <br />Please clarify. If there is, that is, borings on more than 1 parcel, please submit a separate permit application <br />(each parcel needs a separate permit) along with a map and revision on the work plan <br />5) Please confirm (or revise) on the permit application the number the soil borings or temporary wells (DPT, should <br />be 10 based on QAPP) <br />6) Please be aware based on the moneys submitted, $1200, $456 (3 hours at $152) goes to the review fee, and the <br />remaining $744, works out to be 4.89 hours, which is be applied to the existing permit application, unless <br />additional permit application is required (see above) <br />Please provide the above information ASAP so we can process your application <br />Best Regards, <br />Jeff <br />
