If in doubt, contact the Help Desk... <br />please visit our website, and look under Forms: <br />https://www.s*gov.org/department/envhealth/forms <br />Click on Site Mitigation tab, there you'll find all the permit forms you would need for your Phase II Site Assessment or <br />other Site Mitigation endeavor, as well as a guidance. But in brief, you would need to provide to the EHD the following: <br />1. Completed Master File Record, especially the billable party information. This will also act as a service request to <br />review your work plan/ proposal <br />2. Completed Site Mitigation Well/boring application for each separate parcel (APN) <br />3. Work Plan complete with map of where the borings/ well are to be installed, and construction diagram of any <br />proposed monitoring wells <br />4. If this work is required by a lead agency (i.e. EPA, RWQCB, DTSC, etc.), documentation that they are approving <br />this proposed work <br />5. Assess agreement if the work is being done on a property that does not belong to your client <br />6. Fees. Minimum $456 for the plan check, and please contact me for the fees of the well permit (average <br />prepayment per permit is around 3 hours, at $152/hour, hence $456) <br />7.
