See attached manifest. A 5 gallon container was <br /> labelled with a proper hazardous waste label and left onsite for staff to utilize for oil disposal. <br /> #109- Atlas Environmental has submitted all hazardous waste manifests to the State for as long as we have <br /> been a service provider for the hospital. Our office staff has stated that both manifests in question were <br /> submitted to the state immediately after the waste was shipped. We have resubmitted 012218751FLE and <br /> 012218405FLE to the DTSC. Manifests will continued to be mailed in a timely manner moving forward. <br /> #110/#111- Atlas will provide electronic copies of all manifests, 2017-2019, in separate email. In order to <br /> eliminate the manifests ending up in a different/incorrect department upon mailing to Doctors Hospital, Dan <br /> Martin's name will be added to the manifest as the contact. <br /> #605- Please see the attached pictures of the corrected labels for the waste streams in question.
