SAN J D AR I N Environmental Health Department <br /> Y AMENDED <br /> 10/4/2019 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOCTORS HOSPITAL OF MANTECA 1 1205 E NORTH ST, MANTECA October 03, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 102 CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br /> OBSERVATIONS: A black plastic tote labeled used oil was observed in the engineering building containing five 2.5 <br /> gallon containers of hydraulic and pump oil each contained within individual plastic bags.There were no hazardous <br /> waste labels on the 2.5 gallon containers, plastic bags, or the plastic tote. <br /> REGULATION GUIDANCE: Any person who generates a waste shall determine if the waste is a hazardous waste <br /> as specified in Title 22 California Code of Regulations(CCR). There are wastes that are listed as hazardous <br /> wastes. There are wastes that exhibit one or more of the hazardous waste characteristics:toxic, corrosive, reactive <br /> or ignitable. <br /> CORRECTIVE ACTION: Immediately make a hazardous waste determination for the oil, and manage it according <br /> to Title 22 CCR. Use Safety Data Sheets(SDS),waste sampling and test results or other knowledge to support your <br /> hazardous waste determination. Waste testing must be done using methods specified in Title 22 CCR including <br /> sections 66261.20-24. <br /> Submit a statement and supporting documentation with your hazardous waste determination. Demonstrate current <br /> storage and labeling for wastes determined to be hazardous wastes. Provide disposal records for wastes <br /> determined to be hazardous wastes and stored longer than the number of days specified in Title 22 CCR for your <br /> hazardous waste generator status. <br /> This is a minor violation. <br /> 109 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> OBSERVATION: Generator copies of Uniform Hazardous Waste Manifest(UHWM) numbers 012218751 FLE from <br /> 4/9/19 and 012218405FLE from 9/13/18 were not sent to the California Department of Toxic Substances Control <br /> (DTSC)within 30 days of hazardous waste shipment. These two manifests were found in the facilities records but <br /> were not available on the hazardous waste tracker. Facility personnel were unaware of the requirement to send <br /> generator copied of uniform hazardous waste manifests to DTSC. <br /> REGULATION GUIDANCE: Hazardous waste generators are required to send a copy of the generator signed <br /> UHWM to DTSC by mail.The only exception is when an UHWM starts and finishes as an electronic manifest in the <br /> e-Manifest System. The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br /> legible copy of each manifest used.The mailing address is: DTSC Generator Manifests, Department of Toxic <br /> Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br /> CORRECTIVE ACTION: Immediately mail a copy of the listed UHWMs and ensure that copies of all generator <br /> signed UHWMs are sent to DTSC within 30 days of shipping the hazardous waste.
