F; <br /> Site Investigation and No Further Action Request Reporl <br /> R&L Diesel Services, Project#051111 <br /> 4.0 NO FURTHER ACTION REQUEST Slrrce 1954 <br /> _ This No Further Action Request(NFAR) is made in compliance with the Tri-Regional <br /> Board Staff Recommendations for Preliminary Investigation and Evaluation of Underground <br /> Tank Sites dated 16 April, 2004 (Appendix A— Reports). Items required in Section 6.6 are <br /> presented below: <br /> 1. Site history and current site conditions. <br /> Please see Sections 1.2 and 1.3 for a summary of site istory and site conditions. <br /> 2. Site geology and hydrogeology <br /> I <br /> r Please see Sections 3.2.1 and 3.2.2 for a discussion of site geology and hydrogeology. <br /> P <br /> 3. Sensitive potential receptors including water supply wells and surface waiter. <br /> r � <br /> Please see Section 3.4.1 for a discussion of sensitive receptors. <br /> 4. Provide a map showing the location of all municipal domestic, agricultural, industrial and <br /> other uses within 2000 ft. of the site. Provide well df tails and distances in a table. <br /> The sensitive receptor survey did not locate any munici Dal, domestic, agricultural, industrial or <br /> other wells within 2000 ft. of the site. <br /> 5. Provide scaled site maps of the area impacted, sho ' g locations of former and existing ' <br /> F, <br /> tank systems, excavation and sample locations, bot'ng and monitoring well locations, i <br /> groundwater elevation contours, subsurface utilities buildings, streets, and nearby surface <br /> waters. <br /> i <br /> Figure 2 shows the site, including the location of the U T closed in place in 1988, buildings and <br /> streets, and Figure 3 shows the location of all soil borin s within the transmission shop. <br /> K Groundwater elevation is shown on the boring log for B 5 in Appendix B -the only boring which <br /> was advanced to groundwater. No subsurface utilities have been identified in the vicinity of the <br /> UST location. No groundwater monitoring wells were ir stalled as a result of this investigation <br /> and all borings were properly abandoned by grouting th m for their entire vertical extent. <br /> 6. Provide boring logs and cross-sections to show site lithology. <br /> The boring log for B-5 is included in Appendix A, which provides lithology for the impacted area <br /> adjacent to the underground tank. <br /> 7.
