Water Connection Offer <br /> To Connect 166 N. Airport Way, Manteca, CA <br /> m� To the City of Manteca's Water Service <br /> "�qW^ Frank's One Stop, Leaking Underground Storage Tank Case No. 390720 <br /> The groundwater beneath your property has been polluted by Methyl tertiary-butyl ether (MtBE). <br /> The Central Valley Water Board has arranged to connect your property to the City of Manteca's <br /> water supply at no cost to you. Please respond to this letter to accept or decline this offer. <br /> MtBE is a drinking water pollutant that was once used as a gasoline additive to improve air quality. <br /> MtBE has been banned in California since 2004. The MtBE pollution that has affected groundwater in <br /> the vicinity of your property came from a leaking underground storage tank at Frank's One Stop (a <br /> former service station) located at 2072 West Yosemite Avenue. Additional information on this case can <br /> be found here: <br /> hftp://qeotracker.waterboards.ca.gov/l)rofile reportasp?alobal id=T0607700558 <br /> MtBE has either been detected in your drinking water well or in wells near your property. The Central <br /> Valley Water Board has secured funding from the State Water Board's Emergency, Abandoned, and <br /> Recalcitrant Site Program to connect your property to the City of Manteca's water service and to <br /> properly destroy your existing well. This would replace your polluted groundwater supply with a safe <br /> and consistent municipal water supply. <br /> If you accept this offer, the Central Valley Water Board will arrange to have the residence at 166 N. <br /> Airport Way, Manteca, California, APN 200-140-09 (the "Property") connected to the City of Manteca's <br /> water system in Fall 2016. Although connecting to the City of Manteca's water supply will not cost you <br /> anything, you will then be responsible for paying the regular water supply bills. Please respond to this <br /> offer by selecting one of the following options below. <br /> I, Juan & Margarita Mota, hereby: <br /> ❑ Decline the offer of the Central Valley Water Board to connect the Property to the City of Manteca's <br /> water supply system. I will continue to use my existing water supply well. I have been made aware <br /> that water in the area is known to be impacted by MTBE, bacteria, and nitrates which are known or <br /> suspected to adversely impact human health. I agree that I am responsible for all costs associated <br /> with the operation and maintenance of my well and any related treatment systems. I understand <br /> that the State of California assumes no liability in the event I choose not to be connected to the <br /> available public water system, and I understand that this offer may not be available again due to a <br /> lack of funds. <br /> ❑ Accept the offer of the Central Valley Water Board to connect the Property to the City of Manteca's <br /> water supply system and to properly destroy my existing well. By accepting this offer, I <br /> acknowledge that the State will pay the connection fees and installation costs (estimated to cost <br /> $23,000) and the cost for destroying my existing supply well (estimated cost $11,000), and I also <br /> hereby grant the Central Valley Water Board and its contractor(s) reasonable access to my property <br /> to perform the necessary work. <br /> By: Juan & Margarita Mota <br /> Date: <br />
