Repair the wall and provide photographic evidence of correction to the EHD. <br /> 2. Repair the damaged wall and provide photographic evidence of correction to the EHD. <br /> 3. Repair the hales, crack and scuff marks in Ms. Orsua's procedure area. Provide evidence of correction to the EHD. <br /> 33. All surfaces and objects in the procedure area, including chairs, armrests,tables, countertops,and trays,shall be in <br /> good repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a <br /> smooth, durable, and nonabsorbent finish. HSC 119314 <br /> OBSERVATION: <br /> 1. Tape observed on damaged areas of Daniel Juge's and Jessica Orsua's procedure chairs. <br /> 2. Tape observed on damaged areas of Jon Highland's chair. <br /> 3.An absorbent curtain was observed hanging at the entrance of Ricardo Cardenas' procedure area. <br /> CORRECTIVE ACTION: <br /> 1. Repair or replace the damaged procedure chairs. Provide photographic evidence of correction to the EHD. <br /> 2. Repair or replace the damaged chair in Mr. Highland's room. Provide photographic evidence of correction to the EHD. <br /> 3. Remove or replace the absorbent curtain and provide photographic evidence of correction to the EHD. <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A <br /> written Infection Prevention and Control Plan (IPCP) shall be maintained,followed and updated. HSC 119313 <br /> OBSERVATION: <br /> The facility is no longer using Waste Stream Solutions for sharps waste disposal. The sharps disposal company is not updated <br /> on the IPCP. <br /> CORRECTIVE ACTION: <br /> Update the sharps disposal company information on the IPCP. Train all employees on the updated information. Provide EHD with <br /> an updated copy of the IPCP and training records indicating all practitioners were trained on the new information. <br /> 37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art <br /> without a valid registration and at a permitted facility. A body art facility shall notify the LEA in writing within 30 days of <br /> resignation,termination of a practitioner.A new hire or apprentice must be registered with the EHD prior to performing <br /> body art at the facility. HSC 119312 & 119306 <br /> OBSERVATION: <br /> Jon Highland and Jessica Orsua did not pay for their body art permits until November 8, 2023 (receipt of payment was provided <br /> to the EHD). <br /> CORRECTIVE ACTION: <br /> All permit fees are due prior to July 1 st of every year. Ensure all fees are paid on time. Practitioner(s) and facility shall not operate <br /> without a current permit. <br /> Correct All Violations and Provide Evidence of Corrections to the EHD <br /> by January 1, 2024. <br /> Reinspection on/about: A reinspection fee of$162 per hour may be charged.
