Singh, Sandie [EH] <br /> From: Sandip Singh [EH] <br /> Sent: Friday, October 11, 2019 9:55 AM <br /> To: shootersartwork@gmaii.com' <br /> Subject: Final Inspection <br /> Attachments: Infection Prevention and Control Plan.docx; Bloodborne Pathogen Training 2018 <br /> Updated.doc; Hep B Declination.pdf, Body Art Check List for Consent Form V2.rtf <br /> Hi Jose, <br /> The following is needed for the final inspection: <br /> - Infection Prevention Control Plan (IPCP),you can complete the attached IPCP document. <br /> - Training record for the Infection Prevention Control Plan, all practitioners are required to be trained on the IPCP. <br /> Proof of this can be a sign in sheet indicating the practitioners have been trained on the IPCP. <br /> - Sterilization certificates should be on site for the needles that are being used.The sterilization certificates should <br /> be correlated to the needles (matching lot numbers). <br /> - A contract/written agreement between Visual Changes and a sharps disposal company should be available.The <br /> sharps disposal company can either be a mail-back system or a licensed waste hauler. <br /> - A current Bloodborne Pathogen Training Certificate is needed from Hynek. A list of approved courses is <br /> attached. <br /> - Proof of Hepatitis B vaccination is needed for Hynek and Ericson. Either an immunization record, proof of <br /> immunity or a declination form will be sufficient.Attached is a declination form. <br /> - A consent form, aftercare instructions and a medical questionnaire is needed. Attached is Form B, it details the <br /> requirements of each form.There are examples of each of the forms at <br /> https://emd.saccounty.net/EH/Pages/BodvA[I? p lications.aspx. <br /> - If a refrigerator or microwave is still present in the procedure area, please have it removed.
