Unfortunately, not all of the corrections were <br /> submitted from the last inspection (2/18/20) and not all of the corrections meet the requirements of the Safe Body Art <br /> Act. Please submit/correct the following: <br /> 1. A sterilization certificate and a record of purchase were not submitted for the Big Wasp needles(lot#08/2019). <br /> 2. Training record was not provided on the training of the Infection Prevention and Control Plan (IPCP)to all body <br /> art practitioners.The Infection Prevention and Control Plan can act as a training document, but Hynek also has <br /> to sign and date it. <br /> 3. Instances of an autoclave and steaming equipment were mentioned in the IPCP.To my knowledge,Visual <br /> Changes does not use an autoclave or steamer.The IPCP should be updated to exclude the mention of <br /> autoclaving and steaming. <br /> 4. The consent form and medical questionnaire are still missing information. <br /> - Include the following on the medical questionnaire: <br /> o History of herpes infection at the procedure site. "Herpes" is mentioned on the questionnaire but it <br /> has to state "herpes infection at the procedure site." <br /> o History of hemophilia or other bleeding disorders. "Hemophilia" is mentioned on the questionnaire <br /> but it has to state "hemophilia or other bleeding disorders." <br /> o History of cardiac valve disease.The medical questionnaire asks about"heart condition," but it is <br /> not the same as"cardiac valve disease." <br /> o Current medications. <br /> o Requirements for antibiotics prior to surgery or dental procedures. <br /> o Other risk factors for blood borne pathogens. <br /> - Include the following on the consent form: <br /> o Description of the procedure. <br /> o Description of what the client should expect following the procedure.A description of what the <br /> client should expect following the procedure, including suggested care and any medical <br /> complications that may occur as a result of the procedure. <br /> Please have all corrections available for the next routine inspection, if the corrections are not available then a <br /> reinspection will occur with a fee of$152/hr. <br /> Feel free to contact me with any questions. <br /> Best Regards, <br /> Sandip Singh <br /> Environmental Health Specialist <br /> San Joaquin County Environmental Health Department <br /> 1868 E.
