No food, drink, tobacco product, or personal effects are permitted in the procedure area. The practitioner shall not <br />eat, drink, or smoke while performing a procedure. If a client requests to eat, drink, or smoke, the procedure shall be <br />stopped and the procedure site shall be protected from possible contamination while the client leaves the procedure <br />area to eat, drink, or smoke. A client shall be at least 18 years of age to be offered or to receive a tattoo or permanent cosmetic applications, <br />regardless of prental consent. HSC 119302(a) <br /> <br />OBSERVATIONS: <br />1. Observed a client consent form where the date of birth for the client was mistakenly written with the day they filled out the <br />consent form. No ID copy was taken to be able to verify if said client was over 18 years of age. <br />2. Other client consent forms observed where no photo copy of ID was taken to assure that client is over 18 years of age. <br /> <br />CORRECTIVE ACTIONS: <br />1. Assure that the clients are properly filling out the client consent forms prior to the performance of body art procedures. <br />2. Please maintain photo copies of IDs for clients to verify age prior to performance of body art. <br /> <br />18. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form. The client <br />consent form shall include information on the care of the procedure site HSC 119303(a)(5)(A). <br /> <br />OBSERVATION: <br />1. Client consent from observed with the description of the procedure was not filled out. <br /> <br />CORRECTIVE ACTION: <br />1. Assure that client consent forms are being filled out completely prior to the performance of body art procedure. <br /> <br />33. A body art facility shall have counter surfaces and treys that have a smooth, durable , and non absorbent finish. <br />HSC 119314(b)(6) <br /> <br />OBSERVATION: <br />1. Observed a fur pillow in the corner of the procedure room on the leather bench. - Corrected <br /> <br />35. A body art facility shall have a written Infection Prevention Control Plan, and have records of training and shall be <br />maintained for three years and shall be available for inspection upon request of the enforcement officer. <br /> <br />OBSERVATION: <br />1. Facility did not have an IPCP on site for review. <br /> <br />
