After completing the routine inspection, she noted that she wanted me to <br />return towards the end of April to conduct a closure inspection. Closure inspection conducted on April 30, 2025 and the following <br />was noted: <br /> <br />Notes; <br /> <br />During the inspection, the procedure room was completely removed from all materials associated to body art. Room was empty, <br />and the cabinet under the sink was also checked and found to be empty. A 5021 report was filled out with a closuire statement <br />and practitioner signature to inactivate the facility permit. <br /> <br />Suwichada noted that she would continue to practice body art part time and she will be working at Pink Lotus Beauty Studio <br />(PR0547232). <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
