A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br /> client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. HSC 119315(f) <br /> Sterilization records were not available for review. Submit sterilization certificates for two lots in use in the facility <br /> along with photographs of matching lot numbered items to the EHD by 12/17/2019. <br /> 7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard <br /> symbol and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail <br /> back system. Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) <br /> No evidence of a recent sharps disposal was available during the inspection. At least one sharps disposal is <br /> required per year. Submit evidence of a recent sharps disposal,within the last year,to the EHD by 12/17/2019. <br /> 34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312 <br /> Joel Ruiz current permit was not posted. A copy was provided from the department file. Corrected during the <br /> inspection. <br /> All evidence of corrections are due to the EHD by 12/17/2019. <br /> Failure to provide evidence of correction to the EHD by 12/17/2019 will result in a re-inspection charged at the <br /> department's hourly rate of$152 per hour. All evidence of correction can be submitted to the department by email at <br /> 0easter(o),si4ov.orq <br /> Notes: <br /> 1. Medical questionnaire, after procedure care instructions, and consent forms have the required information. Forms <br /> reviewed were complete with lot numbers. <br /> 2. Evidence of monthly spore testing was available along with sterilization logs and Class V integrators. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
