Each sterilization load shall be monitored with mechanical indicators for time, temperature and pressure, Sterilization shall be <br />accomplished by steam autoclave, manufactured for the sterilization of medical instruments, and pass a Class V integrator test. <br />HSC 119315(b) The Tuttnauer Valueklave does not have both temperature and pressure gauges. When replaced, the <br />autoclave must have both temperature and pressure gauges. <br />4. Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A written sterilization <br />shall be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time and <br />temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of the <br />sterilizer after a failed spore -test. HSC 119315(b) An integrator is missing for an autoclave run on 12/1/18. Barrels were <br />autoclaved in October during the time only single use equipment was to be used. There is only one spore test available <br />for the autoclave. Discontinue use until at least two test results have been obtained. Submit evidence of correction to <br />the EHD by 2/4/2019. (Corrected during inspection.) <br />6. A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br />client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. HSC 119315(f) No records are available for some of the pre -sterilized equipment in <br />inventory. Submit evidence of sterilization for lots in use by 2/4/2019. <br />7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard symbol <br />and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. <br />Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) No records are available for sharps <br />disposal. Have a licensed hauler do a pick-up and submit evidence of disposal to the EHD by 2/4/2019. Then have <br />sharps picked -up at least once per year. <br />9. No food, drink, tobacco product, or personal effects are permitted in the procedure area. HSC 119309 There is a vape pen <br />in the procedure area. <br />11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use <br />paper towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) The <br />soap dispensers in two of the work stations are empty. Submit evidence of correction by 2/4/2019. <br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available for inspection. HSC 119303 Some of the client records are incomplete, some do not have copies of client <br />identification, and some do not have the lot information attached. Effective immediately, complete client documents at <br />the time f the procedure and attach the lot information for pre -sterilized equipment used. The medical questionnaire <br />does not have a statement of when to seek medical care. Submit evidence of correction by 2/4/2019. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, rionabso les, and washable. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
