If the skin is to be shaved, the area shall be washed first with soap and water, and then shaved with a single-use razor. The <br /> razor then discarded must be disposed of into an approved sharps container. Prior to performing body art, the client's skin shall <br /> be treated with an antiseptic solution, antimicrobial, or microbicide, according to manufacturer's instructions. HSC 119308(a) <br /> 18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br /> available for inspection. HSC 119303 <br /> 19. At the completion of the procedure, the practitioner shall provide postprocedure instructions that include all elements listed in <br /> Form B. HSC 119308(b) <br /> 23. All surfaces and objects in the procedure, decontamination and sterilization area that come in contact with the client or the <br /> materials used during procedure, including chairs, armrests, tables, countertops, and trays, shall be immediately cleaned and <br /> decontaminated after each use. HSC 119309 <br /> 27. Products applied to skin prior to tattooing or application of permanent cosmetics, including, but not limited to, stencils, <br /> marking and transfer agents, shall be single use and discarded into a waste container at the end of the procedure. HSC <br /> 119311(a) <br /> 28. Sterilized instrument packs, equipment and jewelry shall be stored in a labeled container or cabinet that is protected from <br /> dust or moisture. Sterile instrument packs shall be evaluated before use; instrument packs shall be discarded or reprocessed if <br /> condition is compromised. HSC 119315 <br /> 29. Only jewelry made of approved material and commercially manufactured inks, dyes, and pigments shall be used. Only <br /> single-use needles and needle bars shall be used in tattooing and the application of permanent cosmetics. Needles and needle <br /> bars must be pre-sterilized. HSC 119310& 119311 <br /> 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br /> The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br /> floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art <br /> facilites are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br /> pro urea shall have lined waste containers. HSC 119314 & 119317 <br /> 33. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good <br /> repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a smooth, <br /> durable, and nonabsorbent finish. HSC 119314 <br /> 34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312 <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br /> Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 <br /> A second consultation inspection will be conducted once <br /> all items listed above have been met. Facility may not open <br /> until the second consultation inspection has been <br /> conducted, all items have been corrected, and all applicable <br /> fees have been paid. <br />
