A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a minimum of 90 days after use. HSC 119315(f) — A sterilization certificate for Gabriel Flores' Mast Pro cartridges with lot number 2021-12-01 was not available. Provide EHD with a sterilization certificate for the instrument mentioned. 13. Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen Training consistent with section 119307. — A current Bloodborne Pathogen Training Certificate was not available for Henry Coronilla. Provide EHD with a current copy of Mr. Coronilla's BBP Certificate and ensure a copy is posted in a conspicuous place at the facility. 18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are available for inspection. HSC 119303 — Not all of the client records were at the facility. Ensure all client records for the last 3 years are at the facility. 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each procedure area shall have lined waste containers. HSC 119314 & 119317— The following was observed: -The garbage cans were not lined in Raul Granados' procedure room and in Henry Coronilla's procedure room (corrected on site). - The wall behind Alexis Castillo-Rocha's procedure chair is in need of paint and the baseboard is in need of repair. - Gabriel Alcauter's procedure room and Robert Vierra's procedure room do not have baseboards. Correct all violations and provide evidence of corrections to the EHD bIL June 7, 2022. Notes: 1. The inspection report was emailed to Marisa Correa-Amaya, facility owner, at marisa frank209la�vahoo.com. 2. Barnett is used for sharps waste disposal. The last disposal was on 10/20/21. 3. The floors in the waiting room, Gabriel Flores' room, Raul Granados' room and the hallway have been replaced. The floor was replaced with laminate waterproof flooring (see diagram). Operator is to contact EHD prior to installing flooring in the future. 4. The results of the last spore test was from March 13, 2022. The facility was closed in the month of April, so no spore test was conducted. Per operator, a spore test was sent to the lab for the month of May.
