a. Types of medical waste generated: <br />The Surgery Center generates the following wastes: <br />Blood and body fluids - liquid blood elements or other <br />regulated body fluids, or articles contaminated with <br />liquid blood or body fluids. <br />Sharps - syringes, needles, blades, broken glass. <br />Surgical specimens - human or animal parts removed <br />surgically or by autopsy which are suspected of being <br />contaminated with infectious agents. <br />b. Storage area with description of storage methods: <br />The Surgery Center's Infectious Waste Protocols (Attachment <br />J1) detail storage procedures, and provide that medical <br />waste shall not be stored on the premises for more than <br />seven days. <br />C. On-site treatment procedures: <br />The Protocols outline procedures for waste sterilized in an <br />autoclave. The Surgery Center also maintains a contract <br />with a licensed hauler in the event of an equipment <br />breakdown. <br />d. Name, Address and Identification of Hauler: <br />See Hauler Information on Attachment #2. <br />e. off site treatment facility: <br />The facility's hauler manages its own waste. <br />f. Limited Quantity Exemption: <br />The facility does not have, and does not intend to apply for <br />a limited quantity exemption. <br />
