Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> -Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words <br /> "Chemotherapy Waste"or"CHEMO"on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(4) <br /> Chemotherapy waste containers missing required labels were found in the Allergy Injection Department(1), <br /> Oncology Department and pharmacy(4),and in the Optometry Procedure Room (1). These violations were <br /> corrected during the inspection. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid <br /> and on the sides so as to be visible from any lateral direction. HSC 118280(c) <br /> Biohazardous waste containers missing the required labels were found in Medicine D(4), in the Ambulatory <br /> Surgery Center(2), and in the third floor Soiled Utility Closet(1). These violations were corrected during the <br /> inspection. <br /> Notes: <br /> 1. The Medical Waste Management Plan (MWMP) has the required information. A copy of the current MWMP was <br /> received during the inspection. Training records were available on the segregation of waste streams. <br /> 2. The biohazard bags in use at the facility meet both the ASTM D-1709 and the ASTM D-1922 standards. <br /> 3. There are four pharmacies in the facility. The pharmacies are located on the first floor(outpatient), second floor <br /> (inpatient), third floor(outpatient), and in the Oncology Department. <br /> 4. The main building, the Women's Health building, and the Mental Health Building were inspected. <br /> 5. The Surgery Department and the Second Floor Pharmacy were not inspected.
