Spore Test Checklist 0 <br />Verified 3M Attest Auto Reader is plugged in for at least 30 minutes prior to start of test <br />Obtain biological indicators <br />Don PPE <br />Verified test pack and control tubes are from the same manufacturer <br />Verified the tubes are not expired <br />Test pack loaded in the center of the medical waste load <br />Place the "Testing in Progress, Do Not Operate" sign on the chamber <br />Ran sterilization cycle <br />Load was cooled and test pack was removed wearing PPE <br />The medical waste was left in the chamber <br />The incubation/readout procedure was followed: <br />Don PPE <br />Open test pack and remove indicator tube <br />Close the biological cap by pressing down <br />Crush the glass tube in the crushing well <br />Hold the tube by cap and tap bottom of vial on countertop until the growth media wets the spore strip at the bottom of the vial <br />Place the tube into the incubator well <br />Cover and close, wait for results (at least 3 hours) <br />❑ Results read <br />Negative results: Control tube will always be red, sterilized tube will be green <br />Positive results: Control tube will always be red, sterilized tube will be red <br />❑ Test is negative; documented all information in the Record Log Book, including: <br />Date <br />Load number <br />Sterilizer number <br />Load contents <br />Tube Lot number/expiration date <br />Time and initials <br />Chemical indicator <br />Incubator well numbers, time in/time out, initials <br />Check "131" box <br />negative under Visual <br />❑ Test is negative; Notified EVS can use the Sanipak and removed sign <br />❑ Test is positive; test will be repeated one time <br />❑ Test is positive; notify EH&S Manager, Chief Engineer and EVS Manager <br />Performed by: Title: <br />Verified by: Title: <br />Date: <br />Date: <br />Developed by A.
