Medical Wasteanag ement Plan <br />Kaiser Permanente — Stockton <br />1/01/09-12/31/09 <br />(c) Onsite treatment facility description, including type of treatment utilized <br />maximum capacity, time and temperature necessary, alternate contingency plan <br />in case of equipment failure. <br />• Medical waste treatment was approved to resume on 6/29/08 by the local <br />Environmental Health Department. 2008 Medical Waste Treatment is <br />performed by the use of a San -i -Pak. <br />• The following is applicable: <br />o Onsite waste treatment occurs by use of a San -i -Pak. <br />o This waste is treated through the use of steam sterilization at our <br />permitted facility. <br />o Standard written procedures have been established for the biological <br />indicators and for the use of the steam sterilizer. <br />o Calibration is performed annually. <br />o Records of procedures will be kept for a period of not less than three <br />years. <br />• The following waste are generated at our facility and are treated/removed <br />accordingly by: <br />Type of Waste <br />Method/Treatment <br />Red Bag/Biohazardous <br />San -i -Pak <br />Sharps (needles, syringes, blades etc.) <br />Stericycle <br />Isolation waste <br />San-i-Pak/Steric cle <br />Pharmaceutical waste <br />Stericycle <br />Pathological and surgical waste <br />Stericycle <br />Trace Chemotherapy waste <br />Stericycle <br />The alternate medical waste hauler is: <br />• Ramos Environmental Services, Reg.
