Waste is stored then picked up weekly by Stericycle. <br /> f)Stericycle,4135 W.Swift Ave, Fresno, CA 93722. Stericycle picks up approximately 245lbs of <br /> hazardous waste from this facility each month. <br /> g)Stericycle, 90 N. Foxboro Drive, North Salt Lake, UT 84054 <br /> h)Our Medical Waste Management Plan provides instructions on how to properly handle a hazardous <br /> spill, proper cleaning of equipment that comes in contact with Medical Waste such as using Cavicide to <br /> wipe down carts, and how to properly segregate the different types of Medical Waste into their proper <br /> containers. <br /> i)Our Medical Waste Management Plan covers how to properly handle pharmaceutical waste. <br /> j)lf normal provider could no longer process our hazardous waste and no replacement company could <br /> be secured within 7 calendar days,then surgeries would be suspended until a replacement waste <br /> processing company was under contract. <br /> k)The above information is complete and accurate. <br />
