HSC 117960- LQG shall file MWMP with the EA. <br /> OBSERVATIONS: <br /> The Medical Waste Management Plan was not available for review. <br /> CORRECTIVE ACTIONS: <br /> A large quantity generator required to register with the EHD shall file with the EHD a MWMP on forms prescribed by the EHD. <br /> Submit a Medical Waste Management Plan to the EDH. Ensure MWMP is on site for review. <br /> 13. HSC 117975-LQG shall maintain treatment and tracking documents for 2 years. <br /> OBSERVATIONS: <br /> 1. Treatment/tracking records were not available from August 9, 2025 to August 20, 2025. <br /> 2. Not all treatment records were available for review. <br /> CORRECTIVE ACTIONS: <br /> 1.A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC 118220- Pathology waste of a human nature shall be disposed of by interment, incineration, or an approved <br /> alternative. <br /> OBSERVATIONS: <br /> Pathology waste is being disposed of in the biohazard waste container in the accumulation room. <br /> CORRECTIVE ACTIONS: <br /> Pathology waste of human nature, suspected of containing a highly communicable disease, shall be disposed of by interment, <br /> incineration, or approved alternative. Train staff on proper medical waste disposal. Provide training record to the EHD. <br /> 25. HSC 118280- Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> Per staff, manifolds (suctions) are placed into the pathology waste container in the accumulation room. <br /> CORRECTIVE ACTIONS: <br /> Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and on <br /> the sides so as to be visible from any lateral direction. Train staff on proper medical waste disposal. Provide training record to <br /> the EHD. <br /> 26. HSC 118285-Sharps waste shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> Empty pharmaceutical vials are being disposed of into the garbage (see PHOTO 1). <br /> CORRECTIVE ACTIONS: <br /> Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or tightly lid <br /> and stored onsite not more than 30 days. Sharps container shall be labeled with the words "Sharps Waste"or BIOHAZARD". <br /> Train staff on proper medical waste disposal.
