Employee Requirements <br />1. All employees ate provided with proper coiiection and handling information specificito the type of <br />hazardous materials or waste for which they will be required to work. A qualified person within the <br />department provides training where handling of the materials will occur. Each Department Manager <br />is responsible to document and maintain individual employee hazardous materials training records <br />for their employees. <br />2. All employees handling hazardous materials must wear the appropriate personal protective <br />equipment (PPE) as indicated by manufacture recommendation of the hazardous substance or <br />waste. Hazardous wastes are segregated and handled separately from medical or pharmaceutical <br />waste. Each generating department is responsible for the collection of hazardous material, making <br />sure a Safety Data Sheet (SDS) accompanies the product and transports it to the designated <br />collection site for that facility. <br />3. Hazardous materials are containerized and stored in a locked on-site location for accumulation and <br />pickup. Approved hazardous materials transporter is contracted to remove hazardous wastes from <br />the hospital. <br />B. Incident Reporting And investigation <br />1. The Safety Officer and Director of Facilities Management and Environmental Services share <br />responsibility for the incident reporting process. The Hazardous Materials and Waste Plan utilizes the <br />Hazardous Chemical Spill Report form (L:1LMHtFORMSlFa61ities Managementtl-lazardous Chemical <br />Spill Repo; t 9-06.doc) to document different types of incidents.
