Gary's phone number is 239-8323. Gary said that in December of 2008 he submitted their LQHE for the upcoming year and paid $641. He is stating that they just received a second packet from Environmental Health. The forms in the second packet are identical to the LQHE he already submitted, but the new packet has about 10 additional pages. He wants to know what the new packet is and if he needs to fill it out. He wants to make sure to do everything correctly so they are in compliance. I checked the medical waste file. A copy of their 2009 LQHE is not in there. There are also no notes on the narrative about their LQHE. The most recent entry is from 1/6/09 by KB—"Filed Permit to Operate valid from 1/1/09 to 12/31/09." The entry before that is from 3/7/08 by KF —"Called Steve Willis of PH to let him know Dr's Hospital was going to have a clinic & didn't know if he needed to be involved." I also checked with Kris and Robert and neither of them are sure what the additional packet is. I told Gary that I would email you (because you are at the CUPA conference) and once I heard from you, I would give him a call. If you think it is something I can explain to Gary, email me the answer and I will give him a call back. If it is something that needs to come from you, you can either call him from the conference directly or I can call him and tell him you will get back to him on Friday when you return to the office.
