V as <br />State of California4isalth and Vieftne Agency <br />C <br />Group 1 Diagnostic studies involving measurement of uptake, dilution, or excretion but not involving imaging. <br />Group 2 Diagnostic studies involving imaging including the use of Xenon 127 and/or Xenon 133 gas. <br />Group 3 Reagent kits for preparation of radiopharmaciticals listed in Group 2. <br />Group 4 Internal therapy not usually requiring hospitalization. <br />Group 5 Internal therapy. requiring hospitalization for purposes of radiation safety. <br />Group 9 Nonhuman use, marker, calibration sources, in vitro test, animal studies and Blood irradiator. <br />10. Radioactive material shall be used only at the following locations: <br />(a) 500 West Hospital Road, French Camp, CA <br />l 1. This* license is subject to an annual fee for §owm of radioactive ioactive material authorized to be possessed at any <br />one time as specified in Item 8 of this license. The annual fee for this license is required by and computed <br />in accordance with Sections 30230-30232 of the California Radiation Control Regulations and is also subject <br />to an annual cost -of -living adjustment pursuant to Section 113 of the California Health and Safety Code. <br />12. The individuals named below are authorized the specific uses of radioactive material described in Items 6, 7, <br />8 and 9 of this license as follows: <br />(a) Douglas McGirr, M.D. Groups 1, 2, 3, and 9. <br />(b) Michael R Rigdon, MD. Groups 1, 2, 3, and 9. <br />(c) Marina S. Soosaipillai, M.D. Groups 1, 2, 3, and 9. <br />(d) Wyman K° Yee, NLD. Groups 1, 2, 3, 4, 5, and 9. <br />(e) Ramesh N. Dharawat, MD. Groups 2 (Cardiology studies only), and 9 <br />13. Except as specifically provided otherwise by this license, the licensee shall possess and use radioactive material <br />described in Items 6, 7, 8 and 9 of this license in accordance with statements, representations, and procedures <br />contained in the documents listed below. TheDepartment! s regulations shall govern unless the statements, <br />representations, and procedures in the licensee's application and correspondence are more restrictive than the <br />regulations. <br />(a) The license renewal application with attachments dated March 31, 1989 signed by Michael N. Smith as <br />modified by letters with attachments dated June 29, 1990, July 12, 1990 and September 18, 1990, all <br />signed by Douglas McGirr, MD. <br />
