Biohazard bag is not placed in a rigid container that is leak resistant, tightly-fitting lid, clean, in good repair and <br /> appropriately labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. HSC 118280(c)–Biohazardous bag containing waste was <br /> observed in a large sharps container located in the Soiled Utility Room (H2764) of 2D Med-Surg (see attachment, <br /> PHOTO 9). Ensure biohazardous bags are placed in a rigid container specifically for biohazardous waste. Train <br /> employees on proper disposal of biohazardous waste, provide training record to the EHD. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly-fitting lid, clean, in good repair and <br /> appropriately labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. HSC 118280(c)–The biohazard waste container in the <br /> Ambulance Sali-port(Sheriff's Jail) has a hole in the bottom of the container(see attachment, PHOTO 21). Replace <br /> the container with an appropriately labeled, leak resistant container with a tightly-fitting lid. Submit evidence of <br /> correction to the EHD. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> - Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or tightly <br /> lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" or <br /> BIOHAZARD". HSC 118285–The sharps container in the clean room of the Outpatient Pharmacy did not have a lid <br /> (see attachment, PHOTO 1). Ensure the sharps container has a tight fitting lid, provide photographic evidence to the <br /> EHD. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> -Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or tightly <br /> lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" or <br /> BIOHAZARD". HSC 118285–The sharps container located in the Emergency Room (outside of Triage Consultation <br /> Room) (see attachment, PHOTO 7), Cubicle B in the Pit Exam Room,the Medical Room of Med-Surg (Room H2772), <br /> 2C Medication Room (H2658), Family Maternity Center(A3407), and the Main Lab(see attachment, PHOTO 13) <br /> contained solid waste.Train staff in proper waste segregation,submit a copy of the training documentation to the <br /> EHD. <br /> 29. Intermediate storage area is not secured or marked with proper signage. <br /> - Intermediate storage area shall be either locked or under direct supervision or surveillance and marked with the international <br /> biohazard symbol or the warning signs, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED <br /> PERSONS KEEP OUT', in English and in another language determined to be appropriate by the infection control staff or LEA. <br /> HSC118307–The warning sign on the door of the outside biohazardous waste storage at the Dialysis Clinic has <br /> faded. Replace faded sign(s). <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged. Page 3 of 4 <br /> EH-11/17 <br />
