i <br />SAN JOAQUIN <br />GENERAL HOSPITAL <br />Department Administration <br />Page 8 of 17 <br />Effective Date December 2008 <br />Date Replaces July 2007 <br />Title of Policy/Procedure Hazardous Materials and Waste <br />• Fire extinguisher <br />• Spill kit <br />• Hand-held two-way radio <br />• PPE <br />Emergency response will be limited to the level of training possessed by the individuals <br />responding. <br />90 Day Accumulation Area <br />Wastes are collected at the 90 day accumulation area under the following standards: <br />Hazardous materials shall be evaluated and segregated the according to Appendix 3. <br />Accumulation containers and containment systems shall be inspected weekly for signs of leaks, <br />deterioration, and accessibility. <br />Containers holding hazardous waste must always be kept closed during accumulation except <br />when it is necessary to add or remove waste. <br />Accumulation start time and duration of hazardous waste storage must be adhered to as <br />summarized in Appendix 4. <br />Disposal Procedure <br />SJGH disposes of hazardous waste at San Joaquin County managed disposal facilities or <br />through a hazardous waste contractor. <br />The hazardous waste contractor shall prepare the manifests for waste shipments. These <br />manifests will be reviewed and signed by the Facilities Manager or delegate. <br />The hazardous waste contractor must be permitted by the DTSC and have obtained an ID <br />number. <br />The hazardous waste contractor must comply with the appropriate DOT requirements. <br />The hazardous waste contractor must ship waste only to facilities authorized to accept the <br />waste type. <br />Record Management <br />The Engineering Department shall be responsible for maintaining hazardous waste <br />management compliance documents, including, but not limited to: <br />• Uniform Hazardous Waste Manifest <br />• Biennial Reports of Hazardous Waste Activity <br />• SJGH Hazardous Waste Report <br />All departments are responsible for maintaining training records on their employees and <br />assuring that copies of those training records are sent to Human Resources to be placed in the <br />employee's personnel file. <br />Emergency Response <br />
