The medical waste management plan (MWMP) does not contain all information listed in HSC 117960. <br />- MWMP shall contain all information listed in HSC 117960. HSC117960 — Submit revised MWMP by 1/2/2017. <br />13. This registered large medical waste generator does not maintain individual treatment records and tracking <br />documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br />records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br />HSC117975(a) — Treatment records were not available for review. Submit treatment records by 1/2/2017. <br />23. Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear -resistant. HSC <br />117630 — Obtain and use biohazardous waste bags that are labeled with ASTM standards D1922 and D1709. <br />Submit evidence of correction by 1/2/2017. <br />25. Biohazardous waste is stored above 32 F onsite for more than 7 days before removal. <br />- A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste <br />above 32 F onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month <br />shall not contain or store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) — Tracking <br />documents for the following dates were not available for review: 2/29/2016, 3/7/2016, and 5/23/2016. Submit <br />evidence of pickup by 1/2/2017. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and <br />appropriately labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, <br />and be kept clean and in good repair. Container shall be labeled with the words `Biohazardous Waste" or <br />"BIOHAZARD" on the lid and on the sides so as to be visible from any lateral direction. HSC 118280(c) — <br />Biohazardous waste containers in both designated accumulation areas were missing biohazard labels. Label <br />containers and submit evidence of correction by 1/2/2017. <br />Notes: <br />1.)
