Thank you for sending the tracking documents for <br /> September, October, and November. Please be aware that these records are required to <br /> be held for three years and shall be available for review during each routine inspection. <br /> The Medical Waste Management Plan you provided is grossly outdated and inadequate <br /> and fails to fulfill the requirements of the code section I provided you with during the <br /> inspection. Please utilize the enclosed "Guidelines for the Medical Waste Management <br /> Plan"to comply with the minimum amount of information required to be submitted by <br /> law. Answer these questions at a minimum and submit a copy of this plan back to me by <br /> January 18, 2001. Be sure the information is current, for instance,we are no longer <br /> called the"San Joaquin Local Health District" and are not located on Hazelton Avenue in <br /> Stockton. BFI is no longer known by that name and is not located at the address you <br /> have listed. The bags you use are not provided by BFI so that information is also <br /> inaccurate. "Infectious Waste" is no longer the correct terminology to be used it is <br /> "Biohazardous Waste".
