a
PUBLIC HEALTH SERVICES
Uo
SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DIVISION : `
Karen Furst, M.D., M.P.H., Health Officer , P
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
December 26, 2000
Bernie Brooks, Environmental Services
Lodi Health Care Center
1120 Sylvia Dr
Lodi, CA 95240
Mr. Brooks,
I received your correspondence on 12-18-00 regarding the Medical Waste Management
routine inspection deficiencies. Thank you for sending the tracking documents for
September, October, and November. Please be aware that these records are required to
be held for three years and shall be available for review during each routine inspection.
The Medical Waste Management Plan you provided is grossly outdated and inadequate
and fails to fulfill the requirements of the code section I provided you with during the
inspection. Please utilize the enclosed "Guidelines for the Medical Waste Management
Plan"to comply with the minimum amount of information required to be submitted by
law. Answer these questions at a minimum and submit a copy of this plan back to me by
January 18, 2001. Be sure the information is current, for instance,we are no longer
called the"San Joaquin Local Health District" and are not located on Hazelton Avenue in
Stockton. BFI is no longer known by that name and is not located at the address you
have listed. The bags you use are not provided by BFI so that information is also
inaccurate. "Infectious Waste" is no longer the correct terminology to be used it is
"Biohazardous Waste". Be certain your information is accurate.
Should you have any questions, please call me at(209)468-3451.
f
Kasey L. Foley, Senior R.E.H.S.
Medical Waste Management Program
Enclosures
A Division of San Joaquin County Health Care Services
