This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> - A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)– Treatment records were unavailable for review. Submit treatment records to EHD by <br /> 1/5/2017. <br /> 30. The designated accumulation area is not secured or marked with proper signage. <br /> - A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked <br /> with warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS <br /> KEEP OUT", in English and in another language determined to be appropriate by the infectious control staff or LEA. <br /> HSC 118310–Add proper signage and submit evidence of corrective action by 1/5/2017. <br /> Notes: <br /> 1.) Facility is now contracting with Daniels Sharpsmart, Inc. for the removal of all medical waste. <br /> 2.) Facility has medical waste every 30 days or less. <br /> 3.) Biohazardous waste bags meet ASTM standards (D1922 +D1709). <br /> 4.) Revise MWMP to reflect medical waste hauler change. <br /> 5.) Submit weight report of medical waste being hauled off to verify less than 201bs a month of medical waste is <br /> generated. <br /> Reinspecion on/about: A reinspection fee of$130 per hour may be charged.
