The medical waste management plan(MWMP) does not contain all information listed in HSC 117960. <br /> - MWMP shall contain all information listed in HSC 117960. HSC117960—Complete MWMP and submit by <br /> 1/16/2016. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> - A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)— Treatment records for waste picked up after 10/28/2015 were unavailable. Submit evidence <br /> of compliance by 1/16/2016. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Biohazard bags containing medical waste which will be sent offsite for treatment shall be marked and cetified by the <br /> manufacturer as having passed ASTM D1922 and ASTM D1709. When collecting medical waste within the facility, <br /> the waste may be placed in a biohazard bag which meets ASTM D1709 provided that when the bag is prepared for <br /> transport offsite, it is placed into a USDOT-approved container line with a biohazard bag that is ASTM D1709 and <br /> ASTM D1922 certified. HSC 117630—Obtain appropriate bags. Submit evidence of compliance by 1/16/2016. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management <br /> plan. Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br /> -Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br /> container or secondary container labeled with the words "HIGH HEAT" OR"INCINERATION ONLY"on the lid <br /> and sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)—Pharmaceutical waste must be <br /> placed in a pharmaceutical waste container and picked up by a medical waste hauler for incineration at least <br /> once a year or within 90 days of the container being full. Submit evidence of corrective action by 1/16/2016. <br /> 28. Reusable pails, drums, dumpsters, or bins used for medical waste are used for solid waste containment without <br /> being properly decontaminated. <br /> - A person shall not use reusable pails, drums, dumpsters, or bins used for medical waste for the containment of solid <br /> waste, or for other purposes, except after being decontaminated by the procedures specified in HSC 118295 and <br /> removal of all medical waste labels. HSC 118305—Remove biohazard symbol from soiled linen containers in <br /> room 10 and any other containers throughout the facility. If a container is labeled with a biohazard symbol, all <br /> waste within must be in a biohazardous waste bag and removed by medical waste hauler for treatment.
