Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br /> The biohazardous waste bags currently in the large containers in the biohazardous storage room do not meet both <br /> ASTM D1709 and ASTM D1922 standards. Replace the bags in the containers with bags that meet both standards prior <br /> to shipping the waste off site. Submit evidence of correction to the EHD by 3/5/19. <br /> 24. Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br /> pursuant to this section. The container shall be properly labeled. HSC 118275 <br /> (1) Pharmaceutical waste was discarded in four sharps containers. <br /> (2) Biohazardous waste was discarded in a sharps container. <br /> (3) Batteries were discarded in a sharps container. <br /> (4) The pharmaceutical waste container was not labeled with the date of first use. <br /> Submit evidence of in-service training on the segregation and containment of solid waste, biohazardous, <br /> pharmaceutical, and sharps waste to the EHD by 315/19. <br /> Submit evidence of in-service training on labeling the pharmaceutical containers with the date of first use to the EHD <br /> by 3/5/19. <br /> 25. Biohazardous waste is stored above 32 F onsite for more than 7 days. <br /> -A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br /> onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br /> store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) <br /> Submit evidence of a change in the frequency of pick-ups of biohazardous waste to once every 7 days to the EHD by <br /> 3/5/19. <br /> Notes: <br /> 1) Barnett Medical Services is the licensed medical waste hauler for biohazardous and pharmaceutical waste. <br /> 2) The small biohazardous waste bags meet both the ASTM D1709 and ASTM 1922 standards. The large bags that only <br /> meet the ASTM D1709 can only be used to collect biohazardous waste in the facility. Biohazardous waste that is <br /> shipped off site must be in bags that meet both the ASTM D1709 and the ASTM D1922 standards. The best practice <br /> would be to only use biohazardous waste bags which meet both the ASTM D1709 and ASTM D1922 standards. <br /> 3) Annual training records were available for review. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
