This registered large quantity generator does not file a medical waste management plan (MWMP) with San Joaquin County <br />Environmental Health Department (EHD). <br />- A large quantity generator required to register with the EHD shall file with the EHD a MWMP on forms prescribed by the <br />EHD. HSC117960-Submit an updated copy within 30 days. <br />23. Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear -resistant. HSC 117630 - <br />Facility is storing biohazard waste in clear plastic bags. Submit evidence within 30 days that biohazard waste is being <br />stored only in red biohazard bags. If evidence is not received within 30 days, a re -inspection will be conducted at a rate <br />of $139/hour. <br />24. Biohazardous waste is not placed in a biohazard bag that is conspicuously labeled. <br />- Biohazardous waste shall be placed in a biohazard bag conspicuously labeled with words `Biohazardous Waste" or <br />"BIOHAZARD". HSC 118275(a)(6) <br />- Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br />secondary container labeled with the words "HIGH HEAT OR INCINERATION ONLY" on the lid and sides, so as to be visible <br />from any lateral direction. HSC 118275(a)(6)(B)-Multiple sharps containers wree observed to have pharmaceutical waste <br />(pills and non -empty bottles). Conduct a training with staff reviewing the proper segregation of pharmaceutical waste <br />and sharps waste. Submit evidence of the training within 30 days. This is a repeat violation. If evidence of the training <br />is not received within 30 days or if this issue is observed during the following routine inspection, a re -inspection will be <br />conducted at a rate of $139/hour. <br />Notes: 1.
