The medical waste management plan (MWMP) does not contain all information listed in HSC <br />117960. <br />- MWMP shall contain all information listed in HSC 117960. HSC117960 <br />The Medical Waste Management Plan on file with the EHD is not current. Whenever the plan is updated, it must be <br />submitted to the EHD. Submit an updated copy of the Medical Waste Management Plan to the EHD by 2/25/19. <br />23. Biohazard bag does not meet the standard set forth in HSC 117630. <br />-Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br />The biohazardous waste bags in use in the facility meet the ASTM D1709 standard but not the ASTM D1922 standard. <br />Biohazardous waste bags that meet only ASTM standard D1709 are only satisfactory for collection within the facility. <br />Biohazardous waste bags used to ship waste off site must meet both ASTM standards D1709 and D1922. The best <br />practice would be to only use bags in the facility that meet both ASTM standards D1709 and D1922. Submit evidence of <br />correction to the EHD by 2/25/19. <br />24) Pharmaceutical waste shall be segregated and contained pursuant to this section. The container shall be properly labeled. <br />HSC118275 The pharmaceutical waste container shall be labeled with the date on the date it is placed in use. Submit <br />evidence of correction and an in-service training to the EHD by 2/25/19. <br />24. Pharmaceutical waste is not segregated for storage. <br />- Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management plan. <br />Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6). Pharmaceutical waste was discarded in <br />the sharps container on station #2, cart #2. Submit evidence of an in-service on the segregation of pharmaceutical <br />waste from sharps and biohazardous waste to the EHD by 2/25/19. <br />All corrections are due by 2/25/19. <br />Notes: <br />1) Biologic Environmental Services and Waste Solutions is the licensed medical waste hauler for the facility for <br />biohazardous- and pharmaceutical waste. Biohazardous waste is picked -up once per week. Pharmaceutical waste is <br />picked -up monthly (three times on average). <br />2) The facility is under new management as of September 2017. Shipping and tracking records are available from then <br />until present. Maintain 2 years of shipping and tracking records for future annual inspections. <br />3) The facility maintains electronic shipping and tracking records. <br />4) Annual training records are available for 2018. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
