A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br />store that waste above 32 F onsite for more than 30 days. HSC 118280(e) (1) <br />Shipping and tracking records were not available for the weeks of 8/14/18 and 11/2/18. Submit evidence of medical <br />waste pick-ups for both weeks to the EHD by 3/6/19. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) <br />The biohazardous waste container in the private room was not labeled on the lid and all lateral sides. (This violation was <br />corrected during the investigation.) <br />The designated accumulation area is not secured or marked with proper signage. <br />- A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked with <br />warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT', in English <br />and in another language determined to be appropriate by the infectious control staff or LEA. HSC 118310 <br />The exterior door to outside biohazardous waste storage has been compromised. Submit evidence of repair to the <br />EHD by 3/6119. <br />Notes: <br />1) Stericycle is the licensed medical waste hauler for biohazardous and pharmaceutical waste. Biohazardous waste is <br />picked -up once every 7 days. Pharmaceutical waste is picked -up at least once every 90 days. <br />2) The Medical Waste Management Plan is current and was available for review. Annual training records were available <br />for review. <br />3) The biohazardous waste bags in use in the facility meet both the ASTM D1709 and D1922 standards. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
